Exception Status

Shell/Anadril/001 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Variable Gauge Stabiliser, Fluid Hammer Certificate of Conformity not Available. Delivery note and test certificate are used and include all serial numbers, etc., for traceability Accepted
Shell/Anadril/002 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Material certification not available. Equipment in inventory released through a controlled procedure Rejected for items procured after 1/4/200

Accepted for items procured before 1/4/2000

Shell/Anadril/003 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Variable gauge stabiliser, fluid hammer Lot numbers not specified on individual component. Lot numbers are traceable through unique serial numbers Accepted
Shell/Anadril/004 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Powerdrive Rotary Steerable Bias Unit and Control unit bevels not within NS-2 tolerances. For bevels to meet, ODs of 2 items would need to be different, exposing the face seal of one item to the annulus. Accepted
Shell/Anadril/005 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Bent Subs No SRG in pin end. The angle of the pin prohibits cutting of the SRG. Accepted as Limited Service
Shell/Anadril/006 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Near Bit Subs No bore back in box ends. Length prohibits the application of bore back without making to tool to long for directional drilling purposes. Accepted
Shell/Anadril/007 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Near Bit Stabilisers No bore back at bit end. Bit end bored for float, can not match BSR without reducing OD of stabilisers severely. Accepted
Shell/Anadril/008 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Powerpulse & Impluse MWD/LWD No SRG or bore back. ID too large to apply SRG or bore back. Large ID allows tools to be secured inside collar Accepted
Shell/Anadril/009 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Driveshafts on all Powerpack mud motors No SRG or bore back. Application of SRG or bore back would lengthen shaft and increase susceptibility to failure Accepted
Shell/Anadril/010 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Rocky back stabilisers No SRG or bore back. Stabiliser too short to conform to minimum fishing dimensions Accepted
Shell/Anadril/011 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Mud motor internal connections, e.g. ACME threads No SRG or bore back. ID of stator tube housing too large to allow SRGs or bore backs. Accepted
Talisman/Pathfinder/001 Kevin Fearnley, OCTG Procter Talisman/Pathfinder Dyna-Drill PDM (Driveshaft Bit Box) Client Third Party Inspector to witness MPI of connection Accepted
Talisman/Pathfinder/002 Kevin Fearnley, OCTG Procter Talisman/Pathfinder Dyna-Drill PDM Dyna-Drills manufactured as of 1st April 1999 onwards, only to be supplied unless hardness tests have been performed and documented. Lack of material certification. Not Accepted
Talisman/Drilltech/003 Kevin Fearnley, OCTG Procter Talisman/Drilltech 6.5/8" Reg Pin x 6.5/8" FH box crossover with 8" OD Crossovers only to be used as limited service, i.e. crossing over from transition pipe (HWDP) to collars and not between colloars. BSRs outside recommended range. Accepted
Talisman/Anadrill/004 Kevin Fearnley, OCTG Procter Talisman/Anadrill 6" Hydraulic Adjustable Stabiliser Client third party inspector to verify on load-outs that this tool configuration is to be used in "near bit" BHA setup. Due to pin weak connection, client third party inspector to witness MPI of connection. Accepted
Shell/Anadril/012 Richard Hailes, Shell Expro Shell Expro UESM Mobiles/Anadrill Non-Magnetic Materials No requirement for SRGs in connection sizes up to and including NC38 Accepted
Talisman/SPS AFOS/005 Kevin Fearnley, OCTG Procter Talisman/SPS AFOS MFCT Clutch Type The MFCT does not have the conventional stress relief features machined to pin and box. Although this tool is classified as non-drilling it still sees rotation, albeit at low cyclic stress and its inspection frequency is very routine. Exception is issued on the following: Thread roots are cold rolled after machining (this should be documented in tool history file). Client third party to witness MPI of connections or to verify suppliers inspectors are competent and certified.

Talisman/Weir Houston Engineers/007 Owen Jenkins, OCTG Procter Talisman/Weir Houston Engineers Drilling Jar API Boreback Lengths Accepted
Talisman/Weir Houston Engineers/008 Owen Jenkins, OCTG Procter Talisman/Weir Houston Engineers Drilling Jar API Pin and Box Tong Lengths Accepted
Shell/Weir Houston Engineers/013 Owen Jenkins, OCTG Procter Shell Expro/Weir Houston Engineers Drilling Jar Supply of jars with old-style shart WHE boreback stress relief features in box connections
Shell/Weir Houston Engineers/014 Owen Jenkins, OCTG Procter Shell Expro/Weir Houston Engineers Drilling Jar Reduced Tong Lengths
Shell/Weir Houston Engineers/015 Grant Stewart Shell Expro/Weir Houston Engineers Drilling Jars and Accelerators Cold-rolling/working of Threads and Stress Relief areas  Accepted
